What’s Next?!
Graduating high school or transitioning to living outside of the home is a significant milestone. Thoughtful planning and resources are essential for a smooth transition. At Wake Forest Neurodivergent Services, we recognize that each individual's needs and aspirations are unique. By collaborating, we can define goals and desires for a fulfilling future.
How we work together:
-
Step 1
Join us for an important meeting to discuss your young adult’s goals and support needs. This collaborative session will provide an opportunity for your young adult and you to share insights and express concerns unique to your circumstances. Your input is invaluable as we create a comprehensive plan that aligns with both your child’s aspirations and their individual needs.
This is a one time session for 3 hours that can be completed virtually or in person. Cost: $350

Step 2
I work to create a detailed actionable plan for a successful transition, whether it involves entering the workforce or navigating daily life changes and independent living. This plan includes specific steps tailored to meet your individual needs and specific goals for a rewarding life defined by you. It encompasses resources such as support services, educational tools, and community connections. Additionally, it provides contact information for relevant professionals and organizations that can assist during the transition period.

Step 3
In four weeks, you can expect to receive the final plan via email. Additionally, a complimentary follow-up will be scheduled three months thereafter to review your progress and address any challenges you may be encountering.
